BHN is seeking a compassionate and dedicated Care Coordinator to join our One Care team. In this community-based role, you will support adults with complex medical, behavioral health, substance use, and social needs by helping them navigate healthcare systems, access community resources, and achieve their personal health and wellness goals. Through person-centered care coordination and strong community partnerships, you will play a vital role in improving health outcomes and enhancing quality of life for the individuals we serve.

What You'll Do
  • Serve as the primary point of contact for individuals enrolled in the One Care program
  • Build trusting relationships through regular in-person, telephonic, and virtual outreach
  • Conduct assessments to identify medical, behavioral health, substance use, and social determinants of health needs
  • Develop, implement, and update individualized care plans in partnership with individuals served and their care teams
  • Coordinate services and supports across healthcare providers, behavioral health agencies, community organizations, and social service systems
  • Assist individuals with accessing healthcare, housing, employment, transportation, benefits, and other essential resources
  • Advocate for individuals served while promoting self-determination and informed decision-making
  • Participate in Integrated Care Teams, collaborating with primary care providers, behavioral health clinicians, family members, and other supports
  • Support transitions from higher levels of care, including hospitals, emergency departments, and detoxification programs
  • Monitor service effectiveness and ensure individuals receive appropriate, high-quality care
  • Respond to urgent concerns and safety issues in collaboration with supervisors, leadership, and emergency services as needed
  • Maintain accurate and timely documentation in electronic health records and program systems
  • Build and maintain strong partnerships with community providers and referral sources
  • Meet program productivity standards, documentation requirements, and contractual expectations


Hourly Rate: $22~$26.40

Qualifications
  • High School Diploma or GED required
  • Community Health Worker Certification and/or Bachelor's Degree preferred
  • Bilingual (Spanish/English) candidates strongly encouraged to apply
  • Minimum of 1-2 years of experience in behavioral health, healthcare, human services, care coordination, or a related field required
  • Knowledge of mental health, substance use disorders, healthcare systems, and community resources preferred
  • Experience utilizing motivational interviewing and person-centered engagement strategies preferred
  • Familiarity with electronic health records and technology systems required
  • Valid driver's license and reliable, legally registered vehicle available during working hours required
