We are seeking a passionate and skilled Forensic Mental Health Clinician to provide community-based services within our PACT Forensic program. What You'll Do: As a Master's Level Clinician in the PACT Forensic program, you will provide comprehensive community-based clinical services including therapy, medication administration, ADL support, and care coordination.
Responsibilities include:
- Conduct comprehensive intake evaluations with clinical formulations and recommendations
- Provide individual, family, couples, and group treatment based on sound clinical theories
- Serve as primary case manager for assigned caseload, completing assessments, treatment plans, and treatment plan reviews
- Provide comprehensive risk and crisis evaluations and take appropriate action to ensure safety
- Complete progress notes, incident reports, medication occurrence reports, and all required documentation in a timely manner
- Assist and monitor activities of daily living while encouraging independence and decision-making
- Provide outreach services and transportation to individuals served for medical appointments, errands, and community activities
- Maintain flexibility to deliver services in homes, hospitals, and other community settings
- Utilize motivational interviewing when working with individuals experiencing ambivalence
- Maintain professional licensure and required clinical education hours
What You'll Need: - Master's Degree in Social Work, Psychology, or Mental Health Counseling required
- Licensure preferred (LICSW, LCSW, LMHC); license-eligible candidates encouraged to apply
- Minimum of six months of human services experience preferred
- Willingness and ability to work with forensically involved individuals within the Sequential Intercept model
- Strong verbal and written communication skills
- Cultural competence in supporting clients from diverse backgrounds
- Valid driver's license and reliable vehicle required
- Must obtain and maintain MAP certification within the first 6 months of employment
Schedule: Monday 8am-4pm | Tuesday 12pm-8pm | Wednesday-Friday 8am-4pm
