Senior CommUnity Care of Kentucky PACE is part of Volunteers of America National Services which serves as the Housing and Healthcare subsidiary of the Volunteers of America parent organization.

Pay: $64,000 - $92,000 annually - based on experience

Schedule: Monday - Friday 8:00am - 5:00pm, occasional evenings and/or weekends possible

*Note this position will work interdisciplinary between our two Centers*

    The Home Care Manager oversees the day-to-day operations and operational effectiveness of the Home Care department to ensure high quality, safe, effective care is provided in conformance with policies, procedures and standards and all governing state and federal regulations. Participates in the Interdisciplinary Team (IDT) Meeting as required, acting as the Home Care representative.
    • Education:
      • Bachelor's Degree or equivalent education and experience.
      • Minimum requirement of C.N.A. certification. Current State Specific certification as a Licensed Practical Nurse, Registered Nurse, Occupational Therapist, Physical Therapist or other licensed professional preferred.
      • Cardiopulmonary Resuscitation (CPR) certification required.
    • Experience:
      • A minimum of one year's experience in working with the frail or elderly population required.
      • Minimum of two (2) year's management experience in a hospital, nursing home or community-based setting.
      • Home health experience preferred.
    • Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
    • Must have a valid driver's license, proof of insurance and have reliable means of transportation.
    Skills/Knowledge:
    • Knowledge of principles, practices standards and techniques of a Certified Nursing Assistant (C.N.A.).
    • Working knowledge of the PACE regulations and state regulations.
    • Broad knowledge base of health care problems of the frail elderly.
    • Knowledge of medical equipment and instruments.
    • Knowledge of common safety hazards and precautions to establish a safe working environment.
    • Skilled in identifying problems and recommending solutions.
    • Skilled in preparing and maintaining records and writing reports and responding to correspondence.
    • Skilled in developing and maintaining department quality assurance.
    • Skilled in establishing and maintaining effective working relationships with participants, staff members and family caregivers.
    • Ability to react calmly and effectively in emergency situations.
    • Working knowledge of local health care and geriatric service networks.
    ESSENTIAL FUNCTIONS:
    • Directly responsible for the operational oversight of all Personal Care Supervisor(s), Home Care Scheduler & Home Care aides including schedule management, and overall operational effectiveness of the department.
    • In coordination with the Director of Clinical Services, performs orientation, initial and annual competencies, and is responsible for evaluating performance of all home care staff and schedulers.
    • In coordination with the Director of Clinical Services, performs quarterly supervisory visits in the home of each participant receiving home care services. At least once per year the Manager of Home Care, educates home care aides on best practices in Home and Day Center caregiving practices
    • Prioritizes utilization of homecare staffing needs to reflect actual enrolled participant's homecare needs.
    • Performs initial, semi-annual & change of status assessments of participants' in their home environment to determine their individual needs and coordinates plan of care with care-giving resources.
    • Coordinates with the Interdisciplinary Team to develop plan of care for each participant.
    • Ensures homecare staff follow the participant's plan of care as approved by IDT.
    • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participates in Interdisciplinary Team meetings, communicates participant changes, collaborates on plan of care decisions and coordination of twenty-four (24) hour care delivery.
    • Maintains participants' medical record and fulfills charting and reporting requirements per organizational policy.
    • Monitors participant medication adherence, reporting issues or concerns to appropriate clinical and medication department staff.
    • Responsible for maintaining compliance with all State and Federal program requirements, obtaining and maintain certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules for in home services.
    • Implements quality assurance/improvement systems, policies and procedures per established agency wide protocols and standards. Regularly monitors quality of care and service delivered to ensure compliance with established standards.
    • Establishes and maintains a productive working relationship with relevant contracted providers and agency staff.
    • Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains clinical staff to function independently. Ensures that the job responsibilities, authorities and accountability of direct reports are defined and understood. Assists in the establishment and enforcement of organizational policies and procedures.
    • Adheres to and is a role model of behavior consistent with VOANs Code of Conduct.
    • Acts within the scope of his or her authority to practice.
    • Follows all Volunteers of America National Services policies and procedures, including maintaining a safe working environment, and Occupational Safety and Health Administration (OSHA) safety guidelines.
    • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
    • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
    • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
    • Performs other duties as required or requested.
