Support Whole-Person Wellness as a Nurse Care Manager - Behavioral Health Healthcare Home
Schedule: Days, 8 hour shifts every Tuesday, Wednesday & Friday
Are you passionate about bringing physical and behavioral health care together to support people on their wellness journey? As a Nurse Care Manager in our Behavioral Health Healthcare Home program, you'll be a key part of a multidisciplinary team providing holistic care to clients with serious mental illness and co-occurring conditions. You'll deliver targeted interventions that help close health gaps and empower individuals to lead healthier, more independent lives.

Key Responsibilities
- Manage a caseload of up to 120 clients enrolled in the Healthcare Home program
- Complete health screenings to assess risk and identify gaps in care
- Develop physical health goals and promote healthy lifestyle habits (nutrition, exercise, tobacco cessation)
- Coordinate preventive care services including PCP and specialist referrals
- Perform metabolic syndrome screenings and post-hospitalization medication reconciliations
- Educate clients and team members on chronic health conditions, prevention, and self-management
- Collaborate with providers, care coordinators, and support teams to ensure integrated, person-centered care
- Attend team huddles, staff meetings, and consults across behavioral health programs (CPRC, SUD, ACT, TAY, STEPS)
- Provide in-person services at assigned clinics and occasional home visits as needed
- Maintain compliance with DMH regulations and Compass Health's quality standards
Requirements, Skills, Knowledge and ExpertiseHigh School/GED required
- Practical Nursing or Associate of Nursing degree required
- Bachelor's degree in nursing preferred
Work Experience/Training/Additional Requirements: - Experience in behavioral health or integrated care settings preferred
- A valid driver's license and agency established minimum automobile coverage required
Licensure/Certification:- Licensed Practical Nurse (LPN) or Registered Nurse (RN) required
