Bring Hope and Healing as an ACT Nurse (RN)


As part of one of our Assertive Community Treatment (ACT) teams, you'll provide compassionate, community-based care that helps individuals experiencing serious mental illness achieve stability and wellness. In this role, you'll combine your clinical expertise with a whole-person, recovery-focused approach-offering both direct nursing care and support that empowers clients to thrive in their own communities.

About ACT/ACT TAY/STEPS
ACT (Assertive Community Treatment)/ACT TAY (Assertive Community Treatment for Transitional-Aged Youth)/STEPS (Shape, Teach, Evolve, Prepare, Support)
programs are a way of providing many types of service to people with chronic mental health disorders. ACT/STEPS teams provide services directly to people in their community. People who access ACT/STEPS services receive the wrap-around psychiatric services, but within the comfort of their own home and community. ACT/STEPS team members are comprised of professionals in the areas of psychiatry, social work, nursing, substance use disorders, supported employment, and peer support so they have the skills they need to meet the many needs of the people they serve. The ACT/STEPS team provides these services 24 hours a day, seven days a week, each day of the year.

Key Responsibilities

  • Partner with clients and the ACT team to develop, implement, and evaluate individualized treatment plans
  • Administer and manage medications under the direction of the team psychiatrist, ensuring safe handling and integration into treatment goals
  • Provide health assessments, education, and coaching on medication use, mental illness, and co-occurring conditions
  • Conduct or assist with comprehensive client assessments and documentation, including treatment plans, progress notes, and safety plans
  • Support clients in accessing and understanding medical appointments and treatment recommendations
  • Participate in daily and weekly team meetings, consultations, and supervision
  • Provide on-call crisis intervention and support during evenings, nights, or weekends as scheduled
  • Maintain compliance with all medication, safety, and infection control protocols (CLIA, OSHA, and agency policies)
  • Provide services in community-based settings and assist with client transportation as needed; driving is an essential function of this position


Requirements, Skills, Knowledge and Expertise

Associate's degree in nursing required

QUALIFICATION(S): WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
• Experience in an inpatient or outpatient psychiatric setting preferred
• A valid driver's license and agency established minimum automobile coverage required

LICENSURE/CERTIFICATION

RN or GN that must pass the MO NCLEX exam within 90 days of graduation required
