Pay Range: $22.84 - $26.06, Dependent on Experience
Schedule: Monday - Friday, 8:00am - 5:00pm, Occasional evenings and weekends

About the Job: Coordinate social, recreational, purposeful and spiritual activities to participants , whose needs include psychological, physical and cognitive challenges. Under the supervision of the Center Manager, and as a member of the Senior CommUnity Care Program assesses the needs, interests and capabilities of participants and develops individualized therapeutic recreation plans. In addition, plans, organizes, directs and participates in a comprehensive therapeutic recreation program tailored to the general and individualized needs and limitations of frail elderly program participants. Provides leadership and support to Recreation Assistant

What You Bring (Requirements)
  • Degree in Therapeutic Recreation of similar field preferred. Three years' experience providing therapeutic programming to frail elders preferred. Will consider equivalent experience in lieu of degree.
  • Minimum two (2) years experience in a management position in a health care setting.
  • If operating company vehicle you must be twenty-one (21) years of age and meet the MVR organization standards.
What You'll Be Responsible For
  • Follows the Therapeutic Recreation philosophy of programming to the highest ability level and assisting, through adaptive programming and services, each PACE participant to reach their individual potential.
  • Manages and develops supportive social, therapeutic, spiritual, cognitive, physical, and recreational programing in the PACE Day Center that meet the interests, needs and capabilities of participants. Ensures incorporation of care planned interventions into the Day Center programming.
  • Advises and supports incorporation of therapeutic recreation interventions into homecare services.
  • Functions as an engaged member of the SCCNKY PACE Interdisciplinary Team. Maintains regular attendance at, and participants in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Conducts an in-person initial comprehensive recreational assessment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant enrolled in SCCNKY PACE.
  • As indicated by the Care Team, conducts in-person re-assessments as needed; reassessments may be required semi-annually, when there is a change of condition/status, or when triggered by the Care Team or IDT.
  • Provides skilled one-to-one therapeutic recreation interventions as indicated by any individual plan of care.
  • Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development, and recommended supplies for in-home use.
  • Partners with Marketing Department to further develop and promote the PACE Day Center and community relationships.
  • Maintains participants' medical record and fulfills Senior CommUnity Care charting and reporting requirements. Adheres to all SCCCO documentation policies.
  • Provides recreational educational and training to support Activity Assistant, aides and other PACE Center staff members, as appropriate
  • Plans and implements activities for dementia-specific population. Oversees daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming and sensory stimulation.
  • Responsible for the creation of the monthly newsletter and ensuring distribution to all participants, caregivers and staff. .
